SHIFT:  Evening.  Classes happen in YMCA locations as well as community sites.

RATE OF PAY:  $12.00/hr to $18.00/hr


This position will be primarily responsible for providing quality leadership and supervision in a disease prevention course including but not limited to the YMCA DPP, Blood Pressure Self Monitoring Program, and Salsa Sabor y Salud while maintaining a supportive environment for participants through communication to participants in and out of group sessions. 


The Facilitator must possess the competence and confidence to administer a safe and engaging program for all participants. They must be responsible and at least 21 years of age and have the ability to cooperate and work with YMCA staff to provide a quality program. They must demonstrate good communication and problem solving skills along with a good understanding of health seekers and the community that they are working with.  Must have YUSA's Disease Prevention Programs certification.  Upon hire, CPR AED, and First Aid certification required.  upon hire, Foundations of Listen First, Orientation to Healthy Living, Principles of Member Health and Wellness, Wellness Center at the Y or Group Exercise at the Y, and YUSA HIPAA trainings required.  Within 90 days of hire, Intro to the Ys Cause and Culture training required.  Experience and training in group facilitation preferred.


This individual must familiarize themselves with YMCA rules and policies.  Since this position is dealing directly with program participants, good verbal communication skills are critical.


This individual needs to be able to lift and carry at least 20 pounds in the form of supplies and books.  If individual must lift and carry more than 30 pounds, they must ask for assistance from another staff member.  This individual may be required to stand for extended periods of time while demonstrating physical stamina and agility.


  1. Leads a disease prevention course following the prescribed curriculum.
  2. Leads small groups and takes responsibility for group dynamics and interactions among participants.
  3. Keeps attendance and completes log sheet following each session.
  4. Facilitates group discussion by using active listening with all participants, expressing empathy and engaging entire group.
  5. Needs to be a leader who firmly but quietly takes charge of the group while not becoming a dominant force.
  6. Shares knowledge of health behavior, disease prevention and the difficulties associated with making a lifestyle change.
  7. Must be able to present health education content, lifestyle related risk factors and the impact of certain behavior and clinical values on risk.
  8. Collects required motivators and barriers inventory and evaluation surveys and submits them to the YMCA of the USA designated data collection site for Quality Assurance purposes.
  9. Create a nurturing and safe environment by reporting suspicious activity that may lead to child abuse or may violate the code of conduct.  Keep supplies organized, maintain and clean facility and care for equipment.
  10. Role models SMART behavior.
